In July, after several months of treating my very painful adhesive capsulitis with cortisone injection, physical therapy, oral steroids, and prescription pain meds, my orthopedist recommended surgery on my shoulder. I really didn't want to go through that and decided to try to get past it on my own. After some online research, I purchased this ultrasound device and began to use it prior to stretching exercises at home. Unlike the physical therapists, I stayed within my comfort range on those exercises. Before long, the pain diminished to the point that I could add strengthening exercises too. My ROM is improving week by week. I was able to stop Ultram and will probably not renew my Celebrex prescription. What part did the ultrasound unit play? If I try to start stretching without using it, it takes a lot longer to get the range of the previous day and there is much more discomfort involved. It probably made the difference in my home exercise program being able to solve my problem.

The treatment plan for your frozen shoulder will depend on the stage and severity of your condition. If you receive help while in the "freezing" stage you will generally be recommended conservative, home and alternative treatments which focus on reducing scar tissue formation and increasing pain-free range of motion.

If you receive help in the "frozen" stage, you may be prescribed some medications in addition to the above. The aim of your treatment will be to ease your pain and stiffness, improve your range of motion and keep your joints as mobile as possible.

The longer you have your symptoms, the longer it will take to recover from your frozen shoulder. If you don't follow a treatment plan you can have this condition for up to 3 years. However, a commitment to rehabilitation has been known to speed the healing process to between 4 - 6 months (if you catch the condition in the early stages). It is very important not to re-injure your shoulder during this time to ensure normal function of shoulder when it heals. Surgery is often considered if after 3 - 12 months of home treatments and alternative therapies you see no improvement.

Scar tissue develops in your shoulder joint as tissue heals. This can lead to frozen shoulder or other painful conditions if left untreated.

Surgical Procedures for Frozen Shoulder

Manipulation Under Anesthesia

In rare cases, if the usual treatment options have been unsuccessful, manipulation under anesthesia may be required. Athough this procedure requires anesthesia, no actual surgery involved, meaning incisions are not made when a manipulation is performed. The doctor moves the arm to break up the scar tissue within and surrounding the joint. This is a painful procedure as it requires tearing of the scar tissue, but you will be able to move your shoulder joint more after the manipulation.

Scar tissue develops in your shoulder joint as tissue heals. Manipulation of the shoulder stretches and tears the scar tissue, freeing the arm from the joint.

Follow up the manipulation with cold compression therapy immediately. Continue with cold compression until the pain and inflammation has been reduced. Use ultrasound therapy as well to reduce the amount of scar tisue that will reform as the torn tissue heals. This will help prevent your shoulder from becoming frozen again. Once inflammation, swelling, and bruising has been calmed you can begin BFST® treatments to maintain healthy tissue in your arm and shoulder, reduce the risk of atrophy, and improve the health of the tendons and ligaments that have been damaged during the manipulation procedure.

Arthroscopic Capsular Release

Alternatively, or in conjunction with a manipulation, an arthroscope can be inserted into the joint to cut through the scar tissue. This procedure is called an arthroscopic capsular release. Surgical capsular release of a frozen shoulder is rarely necessary, but it is extremely useful in cases of frozen shoulder that do not respond to physical therapy or regular exercises done at home. If surgery is performed, immediate physical therapy following the capsular release is of utmost importance. If rehabilitation does not begin soon after arthroscopic capsular release, the chance of the frozen shoulder returning is quite high.

Diagnosing Frozen Shoulder

There are several range of motion movements that can help diagnose adhesive capsulitis. Your doctor may ask you to:

  • raise both your hands straight up in the air towards the ceiling.
  • reach across your chest to touch the opposite shoulder.
  • reach behind you and put your back of your hand against the lower back.
  • reach your hand up towards your shoulder blade.

If there is limited range of motion for one or more of these movements, your doctor will probably recommend further diagnostic testing.

Your doctor may ask for an x-ray to look for fractures, a loss of joint space in the shoulder, bone spurs or calcification in the tendon. He or she may do an MRI to be able to see the soft tissue in your shoulder. This will give a clear picture of the muscles, tendons and ligaments in the shoulder, and will allow him or her to assess any damage. This assessment will determine the treatment options available to you. Treatment options will likely include Physical Therapy, stretching and targeted exercises, therapeutic ultrasound, and Blood Flow Stimulation Therapy (BFST®).
